Minnesota Health Information
This clearinghouse for health information helps individuals compare providers, learn how to be a wise health care consumer, and manage personal health. The site was created by the Governor's Health Cabinet and is maintained by the Minnesota Department of Health.

Roadmap to a Healthier Minnesota
"The road to improved population health begins with increased access to care and a focus on preventing disease and promoting healthy choices. By expanding access to coverage, reorganizing our payment systems to promote better coordination of care, and creating environments that help reinforce healthy daily choices, we will pave the way to a healthier state."

Gallup Healthyways Well-Being Index
Daily survey of U.S. adults collecting data on life evaluation, physical health, emotional health, healthy behavior, work environment, and basic access. Data are reported by state and congressional district.

Older Americans 2012: Key Indicators of Well-Being
"This report provides the latest data on the 37 key indicators selected by the Forum to portray aspects of the lives of older Americans and their families. It is divided into five subject areas: population, economics, health status, health risks and behaviors, and health care."

The Community Guide
Provides evidence-based recommendations about community preventive services, programs, and policies that are effective in saving lives, increasing longevity, and improving Americans' quality of life. The task force is an independent, nonfederal, unpaid body appointed by the Director of the Centers for Disease Control and Prevention (CDC).

Stratis Health's Palliative Care in Rural Minnesota Communities
Stratis Health established this initiative to bring community-based palliative care to rural Minnesota communities. They originally supported 10 rural Minnesota communities and have sense expanded to additional communities. They say "managing a serious illness 'take a village'". The website has rich resources about their approach and impact.

Vibrant Communities Canada
Great source of podcasts and resources for collaborative leadership. "[B]uilding a body of practice that will strengthen our individual skills while making the work of communities collaborating easier and more effective for the entire field. As practitioners, our experience has taught us that, in this work, the journey is as important as the destination. The Well Community Project Overview
This project is a collaboration among Samueli Institue, the Institute for Alternative Futures, and CommonHealth ACTION and if funded by the W. K. Kellogg Foundation. The four year initiative hopes to develop a national framework and tools for developing whole community wellness and resilience.
